7th Grade Student Health Requirements

We are collaborating with the Hawaii State Department of Health (DOH) on ways to protect and ensure the health of our keiki. All new and continuing students must complete the following health requirements prior to attending 7th-grade and pursuant to state law:

  • Physical examination (PE)

      1. Must be completed within one (1) year before attending 7th-grade.

  • Immunizations

      1. Must meet the age, dose, and minimum intervals as described in the DOH Immunization Requirements Summary.

      2. Students may be exempt from immunization requirements for medical or religious reasons.

  • Tuberculosis

      1. If your child has been attending a Hawaii school, they do not need a new TB clearance in 7th-grade.

It is important to plan ahead and schedule medical appointments before the new school year begins.

  • Bring the Student’s Health Record (Form 14) to your child’s medical appointment in order to document the completion of the 7th-grade health requirements.

  • Submit the completed Student’s Health Record to your child’s school before the first day of class.

Parents and legal guardians of 7th-grade students who have not completed PE will be notified of the missing requirement and encouraged to schedule a medical appointment as soon as possible. A 7th-grade student who does not have documentation of all of the required immunizations may be allowed provisional attendance at school with verification of an upcoming medical appointment.

If a child is experiencing unstable housing (per the McKinney-Vento Act) or is an undocumented immigrant, refugee, or seeking asylum (as established by Plyer v. Doe), he/she may enroll and attend class while the school arranges for the transfer of health records or while a Community Homeless Concerns Liaison assists the student in meeting health requirements.
